2025-2026 Fall Sports Physical Information 

Here is information regarding Fall Sports Physicals. All physical information goes on Family ID. Every athlete will still be required to have a doctor’s physical, which needs to take place AFTER May 1, 2025. Physicals dated before May 1, 2025 will not be valid. School physical dates will be May 28, 2025, July 9, 2025 and July 16, 2026. Please contact Cheryl Schonewolf (cschonewolf@swasd.org  ) for an appointment. May 28, 2025 will take place at the high school.  The location for physicals on July 9, 2025 and July 16, 2025 may have to be moved. Please continue to check your email if you choose one of those two dates for a location.

A parent/guardian should register by going to this link:

Follow these steps:

 1. To find your program, click on the link provided by the Organization above and select the registration form under the word Programs. 

2. Next click on the green Register Now button and scroll, if necessary, to the Create Account/Log In green buttons. If this is your first time using FamilyID, click Create Account. Click Log In, if you already have a FamilyID account. 

3. Create your secure FamilyID account by entering the account owner first and last names (parent/guardian), E-mail address and password. Select I Agree to the FamilyID Terms of Service. Click Create Account. 

4. You will receive an email with a link to activate your new account. (If you do not see the email, check your E-mail filters (spam, junk, etc.). 

5. Click on the link in your activation E-mail, which will log you in to FamilyID.com

7.   Once in the registration form, complete the information requested. All fields with a red* are required to have an answer. *** THERE ARE MULTIPLE SECTIONS THAT REQUIRE A PARENT AND A “STUDENT/PARTICIPANT” NAME FILLED IN. The two sections shown below are were most of the mistakes happen in FamilyID .  Please read carefully when you get to them. 
8. *** THESE TWO SECTIONS REQUIRE A PARENT AND A “STUDENT/PARTICIPANT” NAME FILLED IN.  THE PARENT CAN NOT SIGN THEIR NAME FOR THE STUDENT/PARTICIPANT.  THEY WILL NOT BE APPROVED TO PARTICIPATE UNTIL THESE TWO SECTIONS ARE COMPLETED CORRECTLY.

9. Click the Continue button when your form is complete. 

10. You will need to upload the PIAA Physical Page #6, signed by your doctor- this is also attached in the email to your student from the athletic director

12. All physical forms must be submitted no later than the Sunday before the start of practice or heat acclimation.  Any form submitted the first day of practice will require the student/athlete to sit out that practice.  This is a change from past years.
